RMA (Return Material Authorization) Request
If you suspect that your machine is malfunctioning, and is covered within the limited warranty, please complete the attached form - in it's entirety - and submit it to us. We will review the problem description you provide, and contact you as to the solution.
Please read our Warranty Policy prior to sending any items back to us.
If you have had repairs performed and your warranty period has not expired, please follow the instructions below. The RMA order number will allow us and you to track and process your return faster. After you receive your RMA number, please pack the item(s) safely along with a copy of the invoice and ship the item(s) to:

IMPORTANT
Customer is responsible for packaging and returning the goods to us undamaged. Mosquito Control Technology will not be responsible for any transit damage. Please also keep a receipt of your return shipment so you can track it. Mosquito Control Technology is not responsible for package(s) lost in the shipment to us. If it is determined that the repaired machine is covered under our warranty, we will be sending the repaired machine back free of charge, standard UPS Ground. Customers will be responsible for the upgrades to other available and more expensive shipping options.
All returned items MUST have the RMA number clearly marked on the outside of box when shipped back to us.
